Key Responsibilities

  • Makes initial evaluations to assess patient eligibility for hospice services within 48 hours of referral.
  • Identifies patients' physical, psychosocial, and environmental needs, reassessing every 15 days as needed.
  • Develops and coordinates personalized care plans, documenting accurate clinical notes regarding patients' conditions.
  • Collaborates with patients, families, physicians, and other healthcare professionals to provide daily patient care.
Requirements

To excel in this role, candidates should have:

  • A minimum of one year of clinical experience as a registered nurse.
  • Current licensure to practice as an RN in the state where the position is located.
  • CPR certification and a valid driver's license with reliable access to a vehicle or public transportation.
